Positive Response Request Form

Positive Response is a system housed at the North Carolina One-Call Center, Inc. that will allow participating facility owners or their locating contractor to provide status of dig tickets sent to them by North Carolina One-Call.  Status is then made available to the excavator for retrieval.

Name Of Facility
  (The facility name is the name that will be heard on the IVR system or viewed on the web by the person requesting status of the ticket.  For example: the member code displayed on the ticket received by the member would be shown as ABC01, but the actual member name that is heard or viewed when checking Positive Response is ABC Utilities.)
